Little Eagles® Martial Arts classes

TERMS & CONDITIONS
This AGREEMENT is made between the Parent or Guardian named above and The Bushido School of Karate, Inc. ("Bushido"). In considera¬tion of the instruction to be provided by Bushido I agree to pay Bushido for Little Eagles® classes until cancelation (see REFUND AND CANCELATION POLICY). If any tuition is unpaid past the due date there will be an additional charge of $25.00 due in addition to any outstanding balance.

REFUND AND CANCELATION POLICY
The tuition set forth is for pre-arranged instruction and is non-refundable after the effective date of this agreement. Any refunds due before the first class begins will be made less the price of any uniforms or training clothing given at the time of registration. You may cancel this agreement at any time by notifying us in writing 10 days prior to your tuition due date. There are no refunds for any classes that have been missed prior to the request for cancelation.

WAIVER Student, or students parent or guardian, acknowledges that participa¬tion in Little Eagles® martial arts and/or in Bushido’s classes and instruction involve an inherent risk of injury. Student, or students parent or guardian, discharges and agrees to hold Bushido harmless of and from any and all claims whatsoever arising out of such injuries
