I have read and agree to the Agreement for Karate Training. By typing my name below I affirm I am over 18 years of age and have the authority to accept this agreement.

This agreement is made between the above named corporation (“Bushido”), its transfers and or assigns, and the undersigned individual, or his or her parent or legal guardian if the student is a minor (“student”). In consideration of the karate instruction to be provided by Bushido described in the PROGRAM SELECTED paragraph, the undersigned agrees to pay Bushido for karate classes via online subscription or otherwise as authorized by me. The undersigned agrees to pay a late charge of $25.00 for any installment not paid on its due date.

Agreement for services and cancellation of these services

Student, or students parent or guardian, acknowledges that participation in the martial arts and/or in Bushido’s classes and instruction involve an inherent risk of injury. Student, or students parent or guardian, discharges and agrees to hold Bushido harmless of and from any and all claims whatsoever arising out of such injuries. It is strongly recommended that you consult with a physician prior to beginning any exercise program, especially if you are over 35 years of age.

Services for karate instruction are cancellable at any time via the My Account Page-Memberships link. Please read the following carefully. It explains the services we provide and it explains your options for cancellation of these services.

This agreement is made between The Bushido School of Karate, Inc. ("Bushido") its transfers and or assigns, and the registered individual member(s), or his or her parent or legal guardian if the student is a minor (“student”). In consideration of the karate instruction to be provided by Bushido described in the PROGRAM SELECTED paragraph, the student agrees to pay Bushido for karate classes via online subscription or otherwise as authorized by me. The undersigned agrees to pay a late charge of $25.00 for any installment not paid on its due date.

PROGRAM SELECTED
Bushido hereby promises to provide Karate lessons online and/or in person in accordance with Bushido’s “belt requirements”. The Program start date is the date this agreement is signed and will renew monthly until canceled.

REFUND POLICY/CANCELLATION
1) START-UP AND/OR REGISTRATION FEES: Bushido may or may not charge a "sign-up" and/or "registration fee" which, if imposed, is included in your first payment. Any "start-up fee" or "registration fee" would be made evident prior to your purchase and would be seen as a line item on your invoice. Any and all "start-up fees" and/or "registration fees" are non-refundable .
2) TUITION: The tuition set forth is for pre-arranged instruction and is non-refundable, except in the case of cancellation provided herein. Bushido may increase tuition at any time and will endeavor to notify you fourteen days prior to the increase. 3) RIGHTS TO CANCELLATION: You may cancel this agreement at anytime and for any reason through the My Account page of BushidoKarate.com. There are no refunds for any reason whatsoever for unused classes. There are no refunds for prepaid tuition if cancellation is made after the monthly renewal date. Bushido reserves the right to cancel membership at any time and for any reason. In the case of cancellation by us (“Bushido”) you will be entitled to a pro-rated refund if applicable.

The Bushido School of Karate, Inc. encourages a consistent and weekly schedule of classes and expects students to abide by them. Should a Member choose to cancel their Subscription and then wish to reinstate their Subscription in the future, we reserve the right to deny such requests, and that the decision to reinstate or deny is at the sole discretion of The Bushido School of Karate, Inc.

Services for paid events not included in karate instruction, including but not limited to Boot Camps and Tournaments, are cancelable and refundable when the following conditions are met: 1) you must request cancellation and/or refund via e-mail to info@BushidoKarate.com 2) Include your name, date, and phone number 3) Refunded amounts will be as follows a) 80% when canceled 30 days or more before the event b) 50% when canceled 15 - 29 days before the event c) There are no refunds for cancelations made 14 days or less before the event.

What to expect

If we haven’t already spoken with you a representative will call at the phone number you provide. That’s it – you’re ready for your first class! Wear comfortable clothing such as loose fitting sweatpants and a t-shirt with sleeves. You will be barefoot. Masks are required once entering the building and anyone entering the dojo will receive a temperature check.
